Default Where to get REBUILD KIT & HELP ID my Turbonetics with pics

Hi, ive got this turbo some time ago (like 5-6 years ago) along with the full turbo kit for 93-01 Nissan Altima, from turbokits.com!!!!
There is no tag on this turbo, may be it fell off, car is used as a daily driver, turbo put on around 60-80k Kmtrs!! (because waste gate pressure - 8-9psi and no boost controller, so turbo never work hard)

If your not able to figure out what turbo is this, i can probably take it off and make some perticular areas pictures, but rather not because im using a car! If you need measurments i can provide them as well!

WHY: some time ago turbine started to smoke after cold start and if not driven, if the car justs sits idling it could smoke all day! As soon as i drive smoke whent away!!!! Now have huge shaft play, almost touching the housing!

LOOKING FOR: some distributor that can sell me the kit to rebuild it!!! Basically both bearings and a thrust washer! DOING it myself!

The things i know:
T3 flange, .63ar, pretty shure its not balbearing, heres pics:

You'd need to pull the wheel measurements of the unit to tell me what it is but first measure the shaft size. If the shaft isn't 5/16" its a small shaft and there aren't any parts for it. If its a big shaft and non-ball bearing there's a rebuilt kit available on the store. If its a ball bearing big shaft Turbonetics would have to rebuild the unit in house.

Looks like a T04E 57 trim with a T3 Stage 3 turbine.
